IN-HOUSE LOYALTY PAYMENT PLAN
At Cache Valley Pediatric Dentistry we believe that not possessing dental insurance should not prevent anyone from getting the treatment they need. The cost of health and dental care continues to increase each year. In an effort to encourage good dental health and alleviate rising dental costs, we have developed an In-House Payment Program. This plan offers exceptional pediatric dental services and treatments to the uninsured, those that do not qualify for Care Credit, and even those individuals and families looking for a more cost effective alternative to their current dental coverage plan.
WHAT DOES IT COST?
Choose to make annual or monthly premium payments at a deeply discounted fee schedule. For families with multiple children we offer additional discounts. After the first child is enrolled, each additional child is discounted $60/year or $5/month.

Annual Fees are due at the first appointment of the year. If paying monthly, current credit card information will be required. Missed Payments due to insufficient funds with a monthly contract, will result in the plan being instantly cancelled, all debts incurred will be due immediately and continued participation in the program will require annual payment rather than monthly.
Enrollment Date begins on the date the Enrollment Fee is paid and will expire after 364 days.
Children in the same family can enroll at different times of the year.
40% Discount on all treatments.
All treatment must be PAID IN FULL at each visit to keep the plan in effect. No additional discounts or coupons may be applied to treatments.
The exams, bitewing x-rays, and cleanings must occur within the 12 month period of enrollment and cannot be carried over.
It is the patient's/parent's responsibility to make and keep appointments for his/her family members.
A $25 fee will incur for each missed appointment without a 24-hour advance notice.
Patients must sign up under a particular provider for 6-month visit, but coverage will transfer to another provider if further treatment is needed.
Our program is not transferable to another party or uncovered child.
Participation cannot be combined with any other offer or dental plan.
Patient must fulfill the 12-month contract, we will not back date if dental insurance is obtained during that period.
All fees will be based on our practice's fees and not any other dental plan's fees.
