1.

INSURANCE WE ACCEPT

Our office accepts most insurance plans. We also accept payment from most PPOs (Preferred Provider Organization) and indemnity plans. As a preferred provider for many insurance companies, we can provide discounted rates on covered services. If we are not a preferred provider for your insurance company, we will accept payment from them, but we are not bound contractually to lower our rates. However, we do our best to maximize the insurance benefits you are eligible to receive.


2.

INSURANCE BILLING

We will happily file insurance claims on your behalf for each appointment using the most current insurance information you have provided. If your insurance plan coverage changes, it is your responsibility to bring those changes to our attention. Please understand that we cannot become involved in prolonged insurance negotiations. Thus, our policy is that if our office has not received payment from your insurance company within 60 days from the date of service, the balance in full becomes your responsibility.


3.

INSURANCE CO-PAY & DEDUCTIBLE

You will need to be prepared to pay any amount that is determined not payable by your insurance plan, such as deductibles and co-pay percentages. You are responsible to know what your insurance covers, what your deductible amounts are, and how much you pay in co-payments on the day of service. If you have questions about specific insurance coverage, your insurance company is the best resource.
